9. Fee Deferral Form (SUMMER 2011) / Fee Deferral Form (FALL 2011)

These forms are used to request fee deferral based on receipt of such OSAP or other government student loans, and is a special form to be used when authorization of deferral comes from the Innis Registrar's Office. As such, these forms can be used ONLY by Innis students and must be authorized by the Innis Registrar's Office based on our updated OSAP information or based on proof of OSAP (or other loan) funding. Fill out every field except authorization, sign and date and then deliver the form (fax or in person) to the Innis Registrar. (Fax 416-978-5503).

If you are on our list of early OSAP applicants and have no outstanding fees, we will defer your fees. If you aren't sure, call our office to check (416-978-2513). If you are not on the list, you have to provide us with documentation indicating you have applied for student aid and an assessment of how much you are receiving.

Alternatively, you can go to the Admissions & Awards Office (315 Bloor St. West) and get them to authorize a fee deferral using one of their forms, at which point you can bring the form to our office for deferral.
