  • Dental Emergencies: HAEMORRHAGE — Click for Presenters' Bios

    In this program, we will review the physiology of blood clotting, and present the causes of excessive bleeding most commonly encountered in the dental practice.

    Bleeding is an expected response to tissue injury, including both accidental trauma and trauma created by dental surgery. If normal coagulation does not occur in a timely manner, haemorrhage, or severe bleeding may result. In every dental procedure, every effort must be made to minimize bleeding and ensure haemostasis to prevent postoperative haemorrhage.

    Dr. Bettina Basrani

    Dr. Basrani is an Assistant Professor and Assistant Head of Undergraduate Endodontics at the University of Toronto. She started her dental education in 1989 at the University of Buenos Aires, Buenos Aires, Argentina. In 1998 she earned a Specialist in Endodontics, followed by a Ph.D., from Maimonides University, Buenos Aires, Argentina. Dr. Basrani research interest include intracanal medication in endodontics.

    Dr. Robert Carroll
    Assistant Dean

    Prior to his appointment as Assistant Dean, Dr. Carroll managed the Professional Practice Department of the Royal College of Dental Surgeons of Ontario and was in private practice for 28 years. While in private practice Dr. Carroll was Chief of Dentistry at Soldiers Memorial Hospital and was responsible for the emergency dental service of that hospital.

    Dr. Cameron Clokie
    Professor of Oral and Maxilofacial Surgery,
    Faculty of Dentistry, University of Toronto

    Dr. Clokie is a Professor of Oral and Maxillofacial Surgery at the University of Toronto. He received his Doctor of Dental Surgery (DDS) in 1985 and subsequently a PhD in bone regeneration related to interface development from McGill University. He has many publications and has lectured internationally on a variety of topics related to oral healthcare including Implant Dentistry and Bone Regeneration. In 1993, Dr. Clokie established the Bone-Integrated Implant Research Group at McGill University that has been actively involved in the evaluation and development of new bone regenerative technologies. In this capacity he has served as a consultant and as a member of the scientific advisory boards for numerous companies and industrial partners. He has been granted patents for technologies developed in his laboratory both for North America and Europe. Dr. Clokie has developed an international network of collaborators working in this field attempting to better understand the impact of growth factor technologies on bone regeneration. The fundamental goal of his work is to develop bioimplants that will eventually replace the need for autogenous bone grafts. He also maintains a busy clinical practice in Oral and Maxillofacial Surgery in Toronto, Ontario with a focus on dental implants, facial reconstruction and bone regeneration.

    Dr. Daniel Haas
    Associate Dean Clinical Sciences,

    Dr. Haas is Professor and Associate Dean at the University of Toronto, Faculty of Dentistry, with a cross-appointment to the Department of Pharmacology at the Faculty of Medicine. He holds the Chapman Chair in Clinical Sciences, is Head the Discipline of Dental Anaesthesia and Director the graduate program in Dental Anaesthesia.

    Dr. Dorothy McComb
    Professor and Head, Restorative Dentistry,
    Director, Comprehensive Care Program,
    Faculty of Dentistry, University of Toronto

    Dr. McComb is Professor and Head of Restorative Dentistry and Director of Comprehensive Clinical Care at the Faculty of Dentistry, University of Toronto. A graduate of the University of Edinburgh, Scotland, she received her Master of Science in Dentistry at the University of Toronto (majoring in Dental Biomaterials) and her Fellowship in Clinical Sciences from the Royal College of Dentists of Canada. Significantly involved in undergraduate teaching, continuing education and biomaterials research since 1980, Dr. McComb has published extensively and lectured widely with over 60 articles, book chapters and major reports authored to date. She participated in the 2001 NIDR Consensus Development Conference on “Diagnosis and Management of Dental Caries throughout Life” producing a systematic review of the evidence pertaining to “The Scientific Basis for the Teaching and Practice of Operative Dentistry 1) The Permanent Dentition and 2) The Primary Dentition” which included 98 references and 5 systematic review tables.

    She is a member of the Editorial Board of the Operative Dentistry Journal, a member of the CDA Committee on Clinical and Scientific Affairs and Dental Sciences Councillor for the Royal College of Dentists of Canada. Dr. McComb is the 2004 recipient of the 3M-ESPE Canadian National Teaching Award and the 2005 W.W. Wood Teaching Award. She has maintained a part-time general practice throughout her academic career.

    Dr. Howard Tenenbaum
    Professor of Periodontology,

    Head Division of Research, Department of Dentistry, Mount Sinai Hospital

    Dr. Tenenbaum received his D.D.S. from the University of Toronto in 1978 and then completed his specialty in Periodontology as well as a Ph.D. in bone cell biology by 1982. This was followed by a 2-year post-doctoral MRC fellowship in Bethesda at the National Institutes of Health. In addition to his degrees, Dr. Tenenbaum has a Fellowship from the Royal College of Dentists of Canada, as well as the International College of Dentists and the Academy of Dentistry International.

    Dr. Tenenbaum is a Professor of Periodontology, and was Head of that Discipline for 8 years (1997-2005), at the Faculty of Dentistry, University of Toronto. He was also Associate Dean for Biological and Diagnostic Sciences for 6 years having just stepped down September 2007 to pursue his research more actively. He is a cross-appointed Professor in the Department of Laboratory Medicine and Pathophysiology, Faculty of Medicine, University of Toronto and is the Head of the Division of Research in the Department of Dentistry at Mount Sinai Hospital. Dr. Tenenbaum also serves as an FDA panel member for Dental Devices and Drugs (USA) and is Vice-Chair of the Federal Dental Care Advisory Committee (Canada).

    Dr. Tenenbaum has had research funding from the Medical Research Council of Canada, CIHR (Canadian Institutes for Health Research) and other agencies for the past 25 years and has pursued ongoing research in the fields of bone cell biology (including a series of bone cell studies aboard the US Space Shuttle carried out by Senator John Glenn), periodontics, and orofacial pain. He has published over 140 articles, book chapters and abstracts in major scientific and clinical journals and has lectured nationally and internationally on several topics including orofacial pain, cellular biology of bone, and management of refractory periodontal diseases.

    In this program, we will cover the principles of treating infection and cover many common infections that may present as an emergency in a dental office.

    Oral infections can be odontogenic in nature or periodontal. If not treated properly and promptly, oral infections may become life threatening situations. Of primary concern is the prevention and management of advanced infection.

    This program looks at the protocols specific to paediatric dentistry including primary tooth abscess, injury to primary teeth and injury to young permanent teeth.

    A paediatric dental emergency is an unpredictable event encompassing any crisis situation in which a child is in pain or discomfort or where a parent has a specific concern.

    Dr. Gajanan (Kiran) Kulkarni
    Associate Professor of Paediatric and Preventive Dentistry,
    Faculty of Dentistry University of Toronto

    Dr. Kulkarni is a Tenured Associate Professor in the Department of Pediatric and Preventive Dentistry at the University of Toronto. He is also a consultant at the Hospital for Sick Children. Dr. Kulkarni received his Dental Surgery education at the University of Bombay, followed by a Bachelor of Laws at the University of Poona. In 1987 he moved to Toronto and earned his Master of Science degree from the University of Toronto, followed by a Diploma in Pediatric Dentistry. He completed his Ph.D. Doctor of Philosophy in 1996 from the University of Toronto. In 2000 he was a Fellow of the Royal College of Dentists of Canada. Dr. Kulkarni is a Diplomat on the American Board of Pediatric Dentistry.

    Over the past ten years Dr. Kulkarni has taught Pediatric Dentistry, Preclinical Pediatric Dentistry, Oral Medicine and Pathology, Preventive Dentistry and Clinical Pediatric Dentistry at University of Toronto.

    Recent research interest includes: molecular mechanisms in craniofacial anomalies of Down's Syndrome and investigating candidate genes causative for congenitally missing teeth in the EL mouse model: Differential gene expression, mutation analysis, and apoptotic processes leading to congenital absence of third molars.

    In this program, you will learn about the most common reason for a patient to call and request urgent care – pain in the orofacial region.

    Classification and diagnosing pain is essential in its successful resolution. This program will also summarize toothaches of various origin including: pulpitis, necrotic pulp, periodontitis, abscess, post operative pain, and non-odontogenic pain.

    In this program, you will review assessing the nature and urgency of a dental emergency due to trauma or accident to the teeth. You will also review documenting patient accident history, medical history and performing clinical examination, treatment and follow up for trauma situations.

    Occasionally, trauma and injury occurs as a result of treatment. This session will also present injury avoidance basics, review the types of injuries that may occur in day to day practice and suggest helpful advice on avoiding these situations.

He received his Doctor of Dental Surgery (DDS) in 1985 and subsequently a PhD in bone regeneration related to interface development from McGill University. He has many publications and has lectured internationally on a variety of topics related to oral healthcare including Implant Dentistry and Bone Regeneration. In 1993, Dr. Clokie established the Bone-Integrated Implant Research Group at McGill University that has been actively involved in the evaluation and development of new bone regenerative technologies. In this capacity he has served as a consultant and as a member of the scientific advisory boards for numerous companies and industrial partners. He has been granted patents for technologies developed in his laboratory both for North America and Europe. Dr. Clokie has developed an international network of collaborators working in this field attempting to better understand the impact of growth factor technologies on bone regeneration. The fundamental goal of his work is to develop bioimplants that will eventually replace the need for autogenous bone grafts. He also maintains a busy clinical practice in Oral and Maxillofacial Surgery in Toronto, Ontario with a focus on dental implants, facial reconstruction and bone regeneration.

    Dr. Daniel Haas

    Dr. Haas is Professor and Associate Dean at the University of Toronto, Faculty of Dentistry, with a cross-appointment to the Department of Pharmacology at the Faculty of Medicine. He holds the Chapman Chair in Clinical Sciences, is Head the Discipline of Dental Anaesthesia and Director the graduate program in Dental Anaesthesia.

    Dr. Dorothy McComb
    Professor and Head, Restorative Dentistry,
    Director, Comprehensive Care Program,
    Faculty of Dentistry, University of Toronto

    Dr. McComb is Professor and Head of Restorative Dentistry and Director of Comprehensive Clinical Care at the Faculty of Dentistry, University of Toronto. A graduate of the University of Edinburgh, Scotland, she received her Master of Science in Dentistry at the University of Toronto (majoring in Dental Biomaterials) and her Fellowship in Clinical Sciences from the Royal College of Dentists of Canada. Significantly involved in undergraduate teaching, continuing education and biomaterials research since 1980, Dr. McComb has published extensively and lectured widely with over 60 articles, book chapters and major reports authored to date. She participated in the 2001 NIDR Consensus Development Conference on “Diagnosis and Management of Dental Caries throughout Life” producing a systematic review of the evidence pertaining to “The Scientific Basis for the Teaching and Practice of Operative Dentistry 1) The Permanent Dentition and 2) The Primary Dentition” which included 98 references and 5 systematic review tables.

    She is a member of the Editorial Board of the Operative Dentistry Journal, a member of the CDA Committee on Clinical and Scientific Affairs and Dental Sciences Councillor for the Royal College of Dentists of Canada. Dr. McComb is the 2004 recipient of the 3M-ESPE Canadian National Teaching Award and the 2005 W.W. Wood Teaching Award. She has maintained a part-time general practice throughout her academic career.

    Dr. Howard Tenenbaum

This was followed by a 2-year post-doctoral MRC fellowship in Bethesda at the National Institutes of Health. In addition to his degrees, Dr. Tenenbaum has a Fellowship from the Royal College of Dentists of Canada, as well as the International College of Dentists and the Academy of Dentistry International.

    Dr. Tenenbaum is a Professor of Periodontology, and was Head of that Discipline for 8 years (1997-2005), at the Faculty of Dentistry, University of Toronto. He was also Associate Dean for Biological and Diagnostic Sciences for 6 years having just stepped down September 2007 to pursue his research more actively. He is a cross-appointed Professor in the Department of Laboratory Medicine and Pathophysiology, Faculty of Medicine, University of Toronto and is the Head of the Division of Research in the Department of Dentistry at Mount Sinai Hospital. Dr. Tenenbaum also serves as an FDA panel member for Dental Devices and Drugs (USA) and is Vice-Chair of the Federal Dental Care Advisory Committee (Canada).

    Dr. Tenenbaum has had research funding from the Medical Research Council of Canada, CIHR (Canadian Institutes for Health Research) and other agencies for the past 25 years and has pursued ongoing research in the fields of bone cell biology (including a series of bone cell studies aboard the US Space Shuttle carried out by Senator John Glenn), periodontics, and orofacial pain. He has published over 140 articles, book chapters and abstracts in major scientific and clinical journals and has lectured nationally and internationally on several topics including orofacial pain, cellular biology of bone, and management of refractory periodontal diseases.

    New approaches to managing difficult periodontal patients.

    Most patients who have periodontitis respond to routine management, which can include scaling and root planning, periodontal flap surgery (regenerative or respective) and, when treatment fails, the placement of endosseous implants. In general 80% of patients will respond to routine therapy, but the problem remains: what do we do about those who do not respond? Are there new developments currently available or on the horizon that will also later what we now accept as being routine therapy, meaning that more patients will respond to treatment and that treatment might either be less invasive, or more predictable, or both?

    There is mounting evidence that patients who so-called refractory periodontitis might be hyper-immune. As such, novel approaches to managing these patients will likely include host modulation therapy (using a collagenase inhibitor such as Periostat®), anti-inflammatory treatment (in combination with host modulation therapy), both of which can be combined with mechanical treatment (scaling and root planning and /or surgery) to effect better outcomes.

    Learning Objectives:

    • Understand refractory periodontitis and the novel approaches available for better outcomes
    • Review the administration and side effects of both systemic and locally administered antimicrobial (LAAs)
    • Identify the needs for adjunctive topical therapies for the patient when clinical improvements of periodontitis have been observed
    • Identify new trends in therapy, their potential for complications and the potential for a novel approach to managing periodontitis
    • Understand how novel periodontal therapies might impact general health
