Forms for Cinema Studies courses requiring applications are posted here for each upcoming academic sesson. The forms are letter-sized Word (.doc) files and can be completed online and emailed to the Undergraduate Office or printed out and brought to Room 122 at Innis College.
►CIN440H1F - Experiential Learning Application
If you are interested in enrolling in the CIN440H1F course for the Fall 2013 term, please fill out the form and submit it to the Undergraduate Cinema Studies Office at cinema.studies@utoronto.ca by April 23, 2013.
►4th year Course Enrolment Form

If you are interested in enrolling in any of the "F" or "S" section code 4th year seminar courses, please fill out the 4th Year Course Enrolment Form and submit it to the Undergraduate Cinema Studies Office in Room 122 or email the form to cinema.studies@utoronto.ca by July 3, 2012. We will assess them based on need and hopefully that may open up some space in the courses for other students who may need them as well.
